Imirian Ravine - from one of the bosses in there iirc; been sometime since I got the recipe though...

No, it drops in Frost Swamp from the boss inside the tent in the large open area where the road forks off. The first boss you'd encounter unles you get the rare one along the road.
It's possible it only drops in lvl 80 instances though, or I had extremely poor luck with 60+ runs through lvl 4X instances.
